Credentialing

• You must complete the credentialing process first.

• Only after you are credentialed will surgical scheduling requests be considered.

Scheduling Requests

• Scheduling Block time is sent by email to manager@micosmeticsurgerycenter.com for review then is boarded by Manager if approved.

Pre-Operative Requirements

• Patient medical clearance and anesthesia clearance must be completed preferably 2 weeks prior to surgery.

Payments

• Payment is due in full 48 hours after surgery is complete.

• Failure to comply with payment policies will result in suspension of privileges until your account is current. Ongoing noncompliance may result in forfeiture of boarding request privileges.

Compliance & Rights

• All procedures must comply with Quad A criteria and all applicable national, state, and local regulations and laws.

• MiCSC reserves the right to refuse or cancel any procedure at any time, with or without cause.
